After you visit our office, check your email for a link to create a profile in our patient portal. Click on the link in the email and create a username and password.
If you are a new patient, prior to coming to our office or before your telehealth appointment, please print out and complete both the New Patient Registration Form and the Payment Information Form. If you cannot print the forms please email them to tfisher@ccvtsa.com.
